Event Form
| For Duval County Public Schools (DCPS) events only, please use the
DCPS Event Form. (Note:
Adobe Reader is required to view this document.) These events require special signatures and
they are required prior to booking your event. Once all information is filled out and signatures obtained, you may fax
it over to the attention of Rebecca Lambert at (904) 348-5761. |
Client Event Form
Are you not with the DCPS and ready to book your event? Please use the
Client Event Form. (Note:
Adobe Reader is required to view this document.) Once you have filled in each section as
detailed as possible, please fax it to the attention of Rebecca Lambert at (904) 348-5761.
Once your forms have been received and processed by the Operations Staff, you will receive an Event Confirmation/Contract via
e-mail. Thanks again for choosing to have your event with us at the Schultz Center.

Catering Services
| The Schultz Center offers catering services for
events held in the Schultz Center. All catering needs must be arranged
through Chartwells Thompson Dining Services. To arrange your catering needs, contact Stephanie Bond at (904) 348-7228. You may also contact Stephanie via email at stephanie.bond@compass-usa.com. |
